The Illinois Mathematics and Science Academy (IMSA) seeks a qualified licensed individual or firm to provide Athletic Trainer Services for the various IMSA student sport programs. Events will be at IMSA, some offsite contests, and all practices, as reasonably required and agreed upon by the parties during the term of the contract.

Submit 5 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Proposal documents may be obtained by contacting the Purchasing Office. This procurement may be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
