The attached specification document, Addendum 1, is issued to further define the original Invitation for Bid (IFB) package for IMSA15000 Residential Halls (5) Summer Cleaning Project, as issued on Friday May 2, 2014 and is now considered a part thereof. Vendors are instructed to read this entire package before attempting to complete the information required therein. Vendors are to acknowledge this addendum by signing and including the signed copy in Packet 1 of your response.
The following changes are to be included in the bid proposals:
VENDORS MUST REGISTER IN ADVANCE OF THE MANDATORY SITE VISIT VIA EMAIL TO ccarreno@imsa.edu no later than Tuesday May 20, 2014 at 12: 30 p.m. CDT. The email must include attendee’s name and title, the company name, the company telephone, and the attendee’s email address.

The Illinois Mathematics and Science Academy is seeking bids for cleaning five residential halls to prepare for the beginning of school in August 2014. Each building consists of forty eight student rooms, four study rooms, two laundry rooms, eight stairwells, four wing lounges with kitchenettes, three entry foyers and a main lobby. The work will begin on Friday, August 1 2014 and must be completed no later than Sunday, August 10 2014.
The Illinois Mathematics and Science Academy is a state owned and operated facility and the following insurances and wage clauses must be met.
Insurances: Vendor shall, at all times during the term and any renewals maintain and provide a Certificate of Insurance naming the State of Illinois as additional insured for all the required bonds and insurance. See University Supplemental Provisions on page 21 of this document for details regarding Insurances.
Wages: As a condition of receiving payments vendor must: (i) Be in compliance with the contract, (ii) Pay its employees prevailing wages when required by law, (iii) Pay its suppliers and subcontractors according to the terms of their respective contracts, and (iv) Provide lien waivers to the state upon request. See University
Supplemental Provisions on page 21 of this document for details regarding prevailing wage.
