Project Description
Illinois State University is seeking statements of qualifications from architectural and engineering firms interested in providing design and construction observation services for a café-style food venue in the soon to be constructed Student Housing Project on the Illinois State University campus. Work will include the fitting out of approximately 1,300 square feet of space allocated for a café in the Community Center building in the complex. Space will be included for preparation, storage, and serving of food products to customers. Estimated cost of project is approximately $450,000.

Vendor/Bidders may qualify to submit bids or proposals to state universities of Illinois only if they are a legal entity authorized to do business in Illinois prior to submitting the bid, offer or proposal. This applies to both in-state and out-of-state firms.

• In good standing with the Illinois Secretary of State

Any firm or individual who wishes to submit their qualifications in response to this solicitation must comply with the vendor requirements listed above, submit the State Board of Elections (SBE) certificate of business entity registration, complete and submit the Contractor Certifications and Conflicts Form (attached), complete and submit the Contractors Disclosures and Conflicts of Interest form (attached), complete and submit the Vendor/Contractor Use of Subcontractors Form (attached), and complete and submit the Subcontractors Standard Qualifications, Certifications, Representations, & Disclosures Form (attached). If you do not submit the SBE certificate (if required), the completed Contractor Certifications and Conflicts Form, the Contractors Disclosures and Conflicts of Interest form, the Vendor/Contractor Use of Subcontractors form, and the Subcontractors Standard Qualifications, Certifications, Representations, & Disclosures Form your qualifications may not be accepted by the University.
Your statement of qualifications must include the information requested above, the information included on the CDB 255 form, and any other information about your firm that you would like to include pertaining to this project. A copy of your CDB pre-qualification confirming your registration, the (1) Certificate of Business Entity Registration from the State Board of Elections, a copy of your (2) Contractor Certifications and Conflicts Form, (3) Contractors Disclosures and Conflicts of Interest form, (4) Vendor/Contractor Use of Subcontractors form, (5) Subcontractors Standard Qualifications, Certifications, Representations, & Disclosures Form, must be included in your response. - Licensed to practice as an architect, landscape architect, or engineer in the State of Illinois.
- Pre-qualified with the State of Illinois Capital Development Board.
- Registered and authorized to do business in the State of Illinois.
Selection will be consistent with the Architectural, Engineering, and Land Surveying Qualifications Based Selection Act (30 ILCS 535/1).
