The Board of Trustees of Illinois State University hereby requests sealed bids for construction work on the alterations to restrooms at Manchester Hall project (Bid # mc21011).
The Board of Trustees of Illinois State University will receive sealed bids for alterations to restrooms at Manchester Hall, Illinois State University, Normal IL until 11:00 a.m. on Thursday February 1, 2001, local time.
Bids are being received at Illinois State University, Purchasing Office, Room 117 Hovey Hall, Campus Box 1220, Normal IL 61790-1220. Bids will be publicly opened and read immediately following the closing time for receipt of bids.
Sealed envelopes containing the bids in duplicate shall be marked:

Bidders shall be prequalified in accordance with Instructions to Bidders issued by the Board of Trustees and made a part of the Project Manual. Bids shall include the following:
1. Bid security in the form of a bid bond, certified check, cashier's check or bank money order, payable to the Board of Trustees of Illinois State University, in the amount of 5% of bid.
2. A letter from a surety company in substantially the form set forth in the Instructions to Bidders assuring the issuance of the Performance and Payment Bonds.
3. One (1) completed copy of the Illinois Department of Human Rights Form PC-2, entitled "Bidder's Employee Utilization Form".
4. Signed Proof of Site Visit Form.
5. One (1) completed copy of the Vendor's Disclosure of Financial Interests.
Bids may not be withdrawn after the opening of bids.

One set of Bidding Documents may be obtained at the Office of the Architect, 108 W Monroe St., Bloomington IL 61702, upon receipt of a plan deposit in the amount of $25, payable to Mills/Sweeney, Architects. Failure to submit a bid and return the bidding documents in good condition withing ten days after receipt of bids will result in the forfeiture of the deposit. Additional sets of the documents may be obtained by payment of printing costs.
Bidding documents shall not be returned with the bid.
Bidding Documents are available for inspection at the Office of Facilities Planning, Illinois State University; Greater Peoria Contractors and Suppliers Association, 1811 W Altorfer Dr, Peoria IL; F. W. Dodge Plan Room, 421 S Grand Ave. W. Suite 1B East, Springfield IL; McLean and Livingston County Construction Council, Inc., 1801 W. Hovey, Suite C, Normal IL; and are on DODGE/SCAN service.
Scope of Project: The project consists of remodeling existing restrooms in Manchester Residence Hall.
Pre-Bid Meeting: 10:00 a.m., Thursday, January 25, 2001, at the site (Manchester Hall). Proof of Site Visit forms will be signed at this pre-bid meeting. THIS IS A MANDATORY PRE-BID MEETING. TO SUBMIT A BID FOR THIS PROJECT, BIDDERS MUST ATTEND AND THE COMPANY NAME MUST APPEAR ON THE ATTENDANCE LOG. BIDS RECEIVED WILL NOT BE OPENED UNLESS BIDDERS NAME APPEARS ON THE ATTENDANCE LOG.
