This Professional and Artistic notice is published in accordance with 30 ILCS 500/35-35 and 44 ILL. Admin. Code 4.2035(e) which allows professional and artistic contracts under $100,000 to be made in accordance with 30 ILCS 500/35-30(f) and 44 ILL. Admin. Code 4.2020 (Small Purchases). The reason this purchase is exempt from the procurement code is as follows: Per the Illinois Nurse Practice Act 225 ILCS 65), it is necessary for nurses, as advanced practice registered nurses, to have a written collaborative agreement with either a family practice or internal medicine physician in order to provide clinical services and prescriptions at our site. It is essential that the collaborating physician be located nearby our office and be available for consultation as needed during our open office hours. Dr. Lazarevic has a private internal medicine practice in Lincoln Square and is available for consultation as needed during our open office hours. Other physicians in the area were contacted after the previous collaborative physician abruptly terminated the service agreement at that time. Dr. Lazarevic was the only physician who agreed to the terms of the service agreement. No other physicians contacted were interested in entering into a collaborative agreement with Student Health Services.

Collaborating physician (Dr. Lazarevic) will formulate guidelines for patient services; periodically review guidelines; provide consultation on an as needed basis; complete patient records; and shall not bill any person for patient services provided.
