Northern Illinois University Health Services requests proposals from responsible vendors to meet its needs. A brief description is set forth in the RFP document, with detailed requirements in subsequent sections of the solicitation. If you are interested and able to meet these requirements, the University appreciates and welcomes a response.

Health Services (hereinafter, "HS") at Northern Illinois University is the on-campus primary care medical practice serving the healthcare needs of its students. HS is fully owned and operated by Northern Illinois University. HS comprises on-site pharmacy, on-site laboratory, on-site radiology (x-ray), Women's Health, Psychiatry, and Medical Services (Primary Care).

Submit 4 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. This procurement may be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. There is no BEP goal for this procurement. The contract(s) for this solicitation could exceed $50,000 annually.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
