Southern Illinois University Carbondale seeks proposal information from qualified physical rehabilitation service providers for the right to service the Student Health Center with a physical rehabilitation operation to include physical therapy services and athletic training services. The University desires to provide convenient, high quality, cost-effective physical rehabilitation services to its students, faculty, staff, and other university guests and constituents.
A mandatory pre-submission conference and site visit will be held to allow Respondents to view the spaces, equipment, and to ask questions. Respondents will be considered non-responsive and disqualified if respondent does not attend, arrives after the meeting is called to order, leaves early or fails to sign the attendance sheet. The pre-submission conference is scheduled for October 4, at 2:00 pm in the Student Health Center located at 374 East Grand Avenue, Carbondale, IL 62901.
