Southern Illinois University Edwardsville is looking for massage therapy services varying from pain management, injury rehabilitation, stress relieve and overall wellness. The University will provide a room located in the Student Fitness Center, equipment, etc. specifically for these services.
Southern Illinois University Edwardsville requires that all massage therapists are responsible for maintaining appropriate certifications and licenses. Acceptable national certifications include: American Massage Therapy Association (AMTA) Certification or Associates Bodywork and Massage Professional (ABMP) Certification. Also, a minimum of 500 clinical hours are required. All therapists must maintain licensure through the State of Illinois as a massage therapist. Additionally, awardee shall maintain insurance with an acceptable carrier, insuring consultant while performing on-site at SIUE as follows: (Minimum) Liability (bodily injury) $250,000 per person, $500,000 per occurrence; Liability (property damage) $100,000 per occurrence.

Submit 4 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. This procurement will be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
