The Board of Trustees of the Southern Illinois University governing Southern Illinois University Edwardsville hereby requests sealed bids to furnish all labor, equipment, tools and materials to remove existing carpeting and wood sub-floor to facilitate installation of floor boxes and conduit to accommodate distribution of power, data, and video in the Student Fitness Center located on the Edwardsville campus of SIUe. Work includes installation of new plywood sub-floor and new rubber sports flooring throughout the cardio fitness area as shown on bidding documents.

All resulting contracts issued against this bid will have required terms to provide full compliance with Illinois Public Act 96-0795,
http://www.ilga.gov/legislation/publicacts/fulltext.asp?Name=096-0795. These terms will apply to the awarded contractor and their subcontractors and include but are not limited to disclosure of financial interests, annual certifications and filing of subcontracts.
Bidders who have not completed and returned their prequalification statement which is valid for the period January 1, 2011 through December 31, 2011 or January 1, 2012 through December 31, 2012, will be required to prequalify in accordance with the bidding contract provision - Document PQ00250 Prequalification Documents. Firms may verify their prequalification status by contacting Shelly at (618) 650-3265. The prequalification document will be due at the SIUE Purchasing Office a minimum of three days prior to the bid opening. Bids will not be accepted from Contractors who have not submitted and received approval of the SIUE Prequalification Statement.
In addition to completing SIUEâs prequalification statement, all bidders may qualify to submit bids to state universities of Illinois only if they are a legal entity authorized to do business in Illinois prior to submitting the bid. All first-tier subcontractors also meet this requirement. This applies to both in-state and out-of-state firms. Failure to provide the Certificate of Good Standing (or other qualifications as noted in the form included with this document) with your bid will result in your bid being considered as non-responsive.
A mandatory pre-bid conference for all those interested in bidding the project will be held on February 21, 2012 at 10:00 a.m. in the Supporting Services Building. Representatives of the University and Hurst-Rosche Engineers, Inc. will be present to answer questions regarding the project and bidding procedures. All prospective bidders are required to attend. All representatives must be in the conference room of the Supporting Services by 10:10 and must stay for the entire meeting and site visit. All contractors are required to sign in and sign out after site visit. If contractor is late or leaves before the completion of the meeting or site visit, the contractor will not be considered attending the pre-bid conference.
A voluntary site visit will be held from 10:00 a.m. to 11:00 a.m. on February 28, 2012. Meet in the lobby of the Student Fitness Center. A building engineer will be available. Attendees must be in the lobby of the Student Fitness Center no later than 10:05 a.m. This is the only additional site visit on this project and will not replace attendance at the mandatory pre-bid meeting. No questions may be asked during this site visit to the Building Engineer or any other SIUe representative.

Answers to all questions will be addressed in the form of an addendum and will be sent to all plan holders and plan houses. Questions should not be addressed to any other SIUE or Hurst-Rosche representative. If contact of any kind occurs with any other representative, this will be grounds for disqualification.
The successful bidder will be required to comply with all laws, statutes, regulations, ordinances, rulings or enactments of any governmental authority which are applicable to the work or to the project. The successful bidder shall pay prevailing wages and shall utilize equal employment opportunity hiring practices in connection with this project.
