Project Description
The Board of Trustees of the Southern Illinois University governing Southern Illinois University Edwardsville hereby requests sealed bids to furnish all labor, equipment, tools and materials to renovate existing 8,500 SF building located in University Park on the Edwardsville campus for the SIUE School of Dental Medicine. Bids will be received from 2016 SIUE prequalified general, electrical, plumbing, heating, ventilation, and fire protection contractors.

Work includes interior renovation, replacement of existing roof top units, a new fire protection system, new medical gas infrastructure, plumbing, heating, ventilating, electrical and technology upgrades. Alternate bid work includes modifications to the existing sidewalks and parking lot as well as parking lot repair and resurfacing.
Bid documents may be obtained at the Southern Illinois University Edwardsville Purchasing Office, Supporting Services Bldg., Room 1020, Campus Box 1012, Edwardsville, IL 62026 or by contacting Shelly Albert at 618-650-3265, Supporting Service Building. Bid documents will be available on or after January 25, 2016, M-F, during the hours of 8:00 ? 12:00 noon and 1:00 p.m. to 3:00 p.m.
A deposit of $200 for one CD and one set of contract documents is required. Suppliers and sub-contractors may receive one set of documents for a non-refundable cost of $100. The deposit must be in the form of a company, cashier?s check, or bank draft payable to Southern Illinois University, Board of Trustees. No personal checks, credit or debit cards, or cash will be accepted. The deposit for the CD and one set of documents will be refunded to bidders submitting bids directly to SIUE upon return of the CD and the contract documents in good condition if not later than thirty (30) days following the bid opening. Fifty percent of the deposit will be refunded to non-bidders who return the contract documents within the specified period. Subcontractors, suppliers and plan houses will not receive a refund.
Firms that require transmittal of plans and specifications via Federal Express or UPS must provide their shipping identification number so that delivery charges can be billed direct to their firm. Plans will not be shipped until the plan deposit/cost check is received.
A mandatory pre-bid conference and one-time only site visit, for all those interested in bidding the project, will be held on January 29, 2016 at 8:30 a.m. at 195 University Park Drive (the facility to be renovated). Representatives of the University and FGM Architects will be present to answer questions regarding the project and bidding procedures. All prospective bidders are required to attend. All representatives must be in the designated room in the 195 University Park Drive Building by 8:40 a.m. and must stay for the entire meeting and site visit. All contractors are required to sign in and sign out after site visit. If contractor is late or leaves before the completion of the meeting or site visit, the contractor will not be considered attending the pre-bid conference. Plans will not be available for pick up at the pre-bid meeting.
Parking is available in the 195 University Park Building and the Technology and Management Center parking lots and along South Research Drive by the Technology and Management Center. Do not park on along the University Park Drive. If you receive a ticket because you are parked on University Park Drive, the parking ticket will be your responsibility.
A one-time only, voluntary site visit will be held at the School of Dental Medicine in Alton in the new Multi-Discipline Lab Building located at 2800 College Avenue, Building 283, Alton, IL, immediately following the completion of the pre-bid meeting in University Park. The Edwardsville Clinic and the MDL, Room 1128, is similar to the scope of work that was done for the larger MDL lab at the Alton Campus. Interested bidders must be at the SDM within 40 minutes of the completion of the mandatory pre-bid meeting and the site visit will last no longer than 30 minutes. No questions may be asked during this one-time only, voluntary site visit. The MDL site visit will not replace the mandatory pre-bid meeting to be held in University Park.
All questions must be e-mailed to Shelly Albert, malbert@siue.edu, by February 3, 2016, 12:00 p.m. Questions submitted after this date may not receive a response. Answers to all questions will be addressed in the form of an addendum and will be posted on the Illinois Higher Education Procurement Bulletin website, www.procure.stateuniv.state.il.us. Questions should not be addressed to any other SIUE or an FGM Architect representative. If contact of any kind occurs with any other representative, this will be grounds for disqualification.
Bidders who have not completed and returned their prequalification statement which is valid for the period January 1, 2016 through December 31, 2016, will be required to prequalify in accordance with the bidding contract provision - Document PQ00250 Prequalification Documents. Firms may verify their prequalification status by contacting Shelly at (618) 650-3265. The prequalification document will be due at the SIUE Purchasing Office a minimum of three days prior to the bid opening. Bids will not be accepted from Contractors who have not submitted and received approval of the SIUE Prequalification Statement.
In addition to completing SIUE?s prequalification statement, all bidders may qualify to submit bids to state universities of Illinois only if they are a legal entity authorized to do business in Illinois prior to submitting the bid. All first-tier subcontractors also meet this requirement. This applies to both in-state and out-of-state firms. Failure to provide the Certificate of Good Standing (or other qualifications as noted in the form included with this document) with your bid will result in your bid being considered as non-responsive.
The successful bidder will be required to comply with all laws, statutes, regulations, ordinances, rulings or enactments of any governmental authority which are applicable to the work or to the project. The successful bidder shall pay current prevailing wage rates as posted on the Illinois Department of Labor?s website and shall utilize equal employment opportunity hiring practices in connection with this project. The University reserves the right to reject any and all bids.
