These labels are for patient encounters. Specifications include: single sheet laser labels, 8 1/2" x 11", #50 White EDP permanent stock; 50# liner; (quality of label must allow for storing in non-temperature controlled environment). Vendor will bid on single sheet laser labels with no ties. Samples must be provided at a minimum of 50 each.

The University reserves the right to accept or reject any and all responses or parts thereof.
