The University awarded a contract for asthma patient outreach, assessment & home visit .
Sinai Urban Health Initiative is providing professional services to improve care for Blue Cross Blue Shield (BCBS) HMO members that are asigned to University of Illinois Hospital and Health Sciences System (UIHHSS) as their BCBS HMO medical site. UIHHSS does not have adequate staffing to provde this additional level of care, so Sinai Urban Health Initiative will provide intensified, individualized intervention to document asthma control and improve asthma management.

There is no other entity authorized by BCBS to provide this service. If services are not performed by Sinai, these patients will not receive this additional level of care.
