Announcement of a Sole Source Purchase

The purpose of this announcement is to modify the amount estimated at the beginning of the term. The University awarded a sole source contract to OrganWise Guys for a comprehensive children’s health and fitness intervention program for grades K-5 based upon gifted funding from Blue Cross and Blue Shield of Illinois. The sole source award of $120,000 had an estimated start date of 4/4/2011 through 3/31/2013. As indicated in the attachment, the success of the educational program yielded additional gift funding. On 10/5/11 a gift of $152,231 was received and present indications are that the program will merit another similar gift. Therefore, the unpredictable award estimate is inadequate to expand the sole source program to the targeted schools.
Consequently this Change Order request is seeking an increase of $300,000 so that the award can be expanded from the $120,000 to $420,000. The estimated cost of the potential renewals is also being adjusted accordingly.

This purchase remains economically available from a single source because the gift funding source specifies this sole source vendor.

The hearing panel will produce minutes of the hearing and may record it in order to aid in the production of the minutes. Interested parties wishing to comment for or against the determination may do so in writing or in person. If you wish to testify, prior registration is requested to ensure adequate time is allowed for all parties.
