The original sole source, 636JCM was submitted for the maximum 18 months. The term will be expiring on 10/31/2017. This authority was requested under a grant that has been extended until 8/31/2018 so that the Legal Council for Health Justice to provide Medical-Legal services for Medicaid patients in Cook County enrolled in the Coordinated Healthcare for Complex Kids (CHECK) program. The Current Agreement executed on 4/20/2016 by the University obligates the aforementioned contract until 10/31/2017.
Therefore this Change Order request is for a revised term date until 8/31/2018 to coincide with the grant extension and an estimated award increase by $25,000 to meet the requirements of the grant.

The University awarded a contract for Medical- Legal services for medicaid patients to Legal Council for Health Justice, Chicago, IL, for an estimated $400,000.
Legal Council for Health Justice to provide Medical- Legal services for medicaid patients in Cook County enrolled in the Coordinated Healthcare for Complex Kids (CHECK) program. Services will consist of: 1) training and support of clinicians to identify and refer patients and their families with health-harming legal needs; legal assistance to patients to obtain necessary resources such as Special Education, Early Intervention, 504 medical accomodation plans at schools, public benefits, healthy housing, Social Security disability, and other state and federal benefits) in order to reduce overall health care costs, increase school attendance, and improve patient engagement.
Legal Council for Health Justice is the only established Medical-Legal Partnership (MLP) for children in the region that has the expertise and capacity to handle the large potential caseload of our 6,000 enrolled Medicaid patients. They have partnered with more pediatric hospitals and health systems than any other provider of legal services in Cook County. They have implemented groundbreaking health policies and programs and are the recipient of many honors and awards including the 2013 Outstanding MLP Awards from the National Center for Medical-Legal Partnership, a national EPA Environment Leadership award for their Westside Children's Asthma Partnership with Mount Sinai Hospital, and a quarterfinalist Collaboration Prize from Lodestar Foundation for work with Friend Family Health Center (FQHC). They handle cases from referral/intake to resolution with in-house attorneys which provides a consistent and long-term solution to our patient's needs. They are also willing to co-locate in our facility to assist our care coordination staff directly regarding new and on-going cases.
The University has determined that this purchase is only economically available from this source because the items are required for research and no other source is able to meet the researcher's documented needs.
