The Board of Trustees of the University of Illinois on behalf of the University of Illinois at Chicago Department of Pathology is seeking Proposals from qualified firms to provide pathology coding services for the period beginning on or about February 1, 2009 through January 31, 2011 with the option to renew for 2 additional twelve-month periods at the same terms and conditions based on satisfactory performance, continuing need and availability of funds.
A pre-proposal conference will be held at the University of Illinois College of Medicine Research Building at 909 South Wolcott, Room 6175, Chicago, Illinois 60612 on December 4, 2008 at 9:00 AM. Prospective Proposers' attendance at the pre-proposal conference is highly recommended as representatives of the University will be present to provide details regarding the services requested and answer any questions regarding the proposal procedures. Prospective Proposers must respond by e-mail at shorter@uillinois.edu or 312-996-2850 on or before December 2, 2008 with firm's name and number of attendees. A maximum of 2 representatives from each firm may attend. Representatives of the University further reserve the right to visit Prospective Proposers sites.

Submit 8 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Proposal documents may be obtained by contacting the Purchasing Office. This procurement will be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
