The Board of Trustees of the University of Illinois on behalf of the University of Illinois College of Medicine at Rockford, Department of Family and Community Medicine is seeking Proposals from qualified Proposers to furnish and dispense all prescription and non-prescription medications for inmates of the Winnebago County Jail for the period beginning on or about June 1, 2009 through June 30, 2012, with the option to renew for two (2) additional 12-month periods at the same terms and conditions based on satisfactory performance, continuing need and availability of funds.

Submit six copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Proposal documents may be obtained by contacting the Purchasing Office. This procurement may be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
