The Board of Trustees of the University of Illinois on behalf of its three campuses, Chicago, Springfield, and Urbana-Champaign, and its multiple off-site locations is seeking proposals from qualified firms to provide Medical Case Management Services for workers�?? compensation claimants. Medical Case Management Services are intended to assist in maximizing medical recovery and facilitating the return to work goals for the period beginning on or about January 1, 2014 through December 31, 2016, based on continuing need and favorable market conditions, when in the best interest of the University. There is no guarantee of maximum utilization or expenditure with any successful respondent during the contract period.

Submit 6 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Proposal documents may be obtained by contacting the Purchasing Office. This procurement may be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. There is no BEP goal for this procurement.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
