UIC on behalf of the University of Illinois Hospital & Health Sleep Science Center is seeking Proposals from qualified firms to provide polysomnography technicians and sleep diagnostic equipment for the nine (9) individual bedrooms clinical sleep laboratory for both clinical and research sleep studies. The studies will be conducted on both adult and pediatric (age six months and older) patients. The clinical sleep laboratory is located at 2244 W. Harrison Avenue, Suite 104, Chicago, IL.
The University will hold an in-person pre-proposal conference at the University of Illinois at Chicago, Hospital Administration, 1740 W. Taylor Street, Suite 1400, Room A, Chicago, IL 60612 at 1:30 p.m. CST on Friday, October 17, 2014. A Representative of the University will be present to answer questions regarding the services requested or proposal procedures. Prospective Proposers shall respond to Ingrid Worrell at (312) 996-2839 or email at iworrell@uic.edu on or before October 15, 2014 with firm name and number of attendees. A maximum of three (3) representatives form each firm may attend.

Submit 5 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents.
Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. There is no BEP goal for this procurement.
