Sealed proposals will be accepted at the address below until 1:30 PM, Thursday June 29, 2000:
for a vision care program for graduate assistants and fellows.
The successful proposer will provide a vision care program to approximately 2,800 graduate assistants and fellows (subscribers), and at the option of the subscribers, to their dependents, through its network of qualified participating vision care service providers. At a minimum, the program is to include an annual comprehensive eye examination, discounts on cosmetic and prescription glasses, and discounts on contact lenses and contact lens fittings.
Proposers must provide a program plan for providing the requested services and must respond to a series of questions in the request for proposal.(Submitted by Thomas L. Gardner, State Purchasing Officer.)
