The Board of Trustees of the University of Illinois, on behalf of the University of Illinois at Chicago and its College of Dentistry, is seeking Proposals from qualified firms to act as the wholesale distributor of clinical dental supplies for the College of Dentistry for the period July 1, 2008 or from the date of execution of the contract, if later, through June 30, 2011 with the option to renew for 2 additional one-year periods at the same terms and conditions based on satisfactory performance, continuing need and availability of funds. The University anticipates an estimated expenditure of $2,000,000 annually for dental products and equipment.
The University will hold a pre-proposal conference at the College of Dentistry located at 801 S. Paulina, Chicago, IL in Room 104E (Dean's Conference Room) at 2:00 p.m. on Tuesday, August 21, 2007. Representatives of the University will be present to answer any questions regarding the services requested or proposal procedures and proposers are encouraged to attend. Prospective proposers must respond to Mike Hallahan by email at hallahan@uillinois.edu or by phone at 1-(312)-996-2432 or 1-312-996-2850 by no later than Monday, August 20, 2007 with the firm's name and number of attendees. A maximum of 3 representatives from each firm may attend. Proposals submitted by companies not in attendance at any conference may not be accepted.

Submit 6 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Proposal documents may be obtained by contacting the Purchasing Office. This procurement will be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
