The Board of Trustees of the University of Illinois on behalf of the University of Illinois Medical Center Ambulatory Services Administration is seeking proposals from qualified firms to provide outpatient patient satisfaction telephone and mail survey services for the period commencing December 1, 2007 through June 30, 2008 with the option to renew for 3 additional twelve-month periods at the same terms and conditions based on satisfactory performance, continuing need and availability of funds.
