The University of Illinois Medical Center at Chicago, is seeking proposals from qualified firms to provide in-house prosthetic and orthotic services. Approximately 1600 sq. ft of dedicated space for the Prosthetic and Orthotic services is available in the hospital. An additional 1300 sq. ft of space is shared with the physical therapy department (waiting room, reception/office and storage). This space is configured for direct clinical services and for limited fabrication of prosthetic and orthotic devices.
The Medical Center will hold a pre-proposal conference at the University of Illinois Medical Center, 1740 W. Taylor, Chicago Illinois 60612 at 8:00am, Central Standard Time on April 7, 2005 in Room C-100 (concourse level). Representatives of the Medical Center will be present to answer any questions regarding the services requested or proposal procedures. Prospective proposers must respond to Mattie McGraw at 312.996.2850 on or before April 6th with firm name and number of attendees. A maximum of 3 representatives from each firm may attend.
