The University will hold a pre proposal conference at the University of Illinois Medical Center at Chicago, 1740 West Taylor Street, Chicago IL 60612, Room 1130, Chicago IL, on October 19, 2009, at 11:30am to 12:30pm. Representatives of the University will be present to answer any questions regarding the services requested or proposal procedures. Prospective Proposers must respond to Ruth Rios by e-mail at riosr@uic.edu or by phone at 312.996.2850 on or before October 16, 2009, with firm name and number of attendees. A maximum of two (2) representatives from each firm may attend.

Submit 4 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Proposal documents may be obtained by contacting the Purchasing Office. This procurement will be contingent on Board of Trustees approval. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
