The Board of Trustees of the University of Illinois, on behalf of Staff Human Resources, is seeking Bids from qualified medical hospitals and/or clinics to provide occupational medical testing services to the University of Illinois at Urbana-Champaign. The services are requested for a two-year initial term, and potential six, one-year renewal options. Only Vendors in Champaign County, Illinois, due to close proximity to the University of Illinois, Champaign-Urbana campus will be deemed responsive. Responsive Vendor means the Vendor is determined to be in substantial conformance with the conditions required in this IFB. Close proximity means: Vendor's facility is located within the Champaign, County, Illinois, U.S.A only. Services may not be performed by mobile units to ensure that University personnel have access to all available equipment, facilities, resources, medical expertise and services that may be necessary for the occupational services and medical testing to be provided. The Vendor must be able to perform these exams 24 hours a day 7 days a week to provide medical testing along with a staffed trauma emergency room staffed 24 hours a day, 7 days a week, in order to provide necessary medical care for employees of the University that work various shifts, emergency and on-call hours, and operate facilities that must be in operation 24 hours a day, seven days a week.

The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Illinois Department of Human Rights
(IDHR) eligibility number is required on all proposals, if applicable. There is no BEP goal for this procurement. The contract(s) for this solicitation could exceed $50,000 annually.
This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
