Request for Proposal: Electronic Veterinary Health Information System
This solicitation will not be awarded based on the Administrative Rules Section 4.2040, "Any solicitation may be canceled before or after opening when the SPO believes cancellation to be in the State's best interest." A new solicitation may be put out at a later date.
Amendment #4 is to extend the proposal opening date to October 6, 2016 at 2:00 P.M.
Amendment #3 is to answer vendor questions and provide clarification.
Amendment #2 is to extend the proposal opening date to September 22, 2016 at 2:00 P.M.
Amendment #1 is to provide the list of participants to the pre-bid conference call on August 4, 2016, and their BEP Certification status.

The University of Illinois College of Veterinary Medicine seeks to procure software which will function as an Electronic Health Information System (HIS).
An optional Pre-Bid Meeting to discuss State Requirements will be held via Conference Call on August 4, 2016 at 2:00 PM CT. Please see section 1.5 for additional information. Preferences in favor of resident bidders may apply. Please see section 2.2.3 of the RFP.

Submit 1 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents.
Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. There is a BEP goal for this procurement. See solicitation documentation for details.
