Amendment 1 clarifies that on page 14 a center perforation is required - no micro-perforation. Also, on page 14 the total number of pages for each of six booklets is eight double-sided printed pages. All other terms of RFP 1SJC1202 apply.

The Board of Trustees of the University of Illinois on behalf of the Center for Prevention Research and Development (“the University”) is seeking Proposals from qualified firms (“Proposers”) to provide the printing of six separate scannable Illinois Youth Survey booklets as defined further in the attached Technical Section.

Submit one (1) original and three (3) copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof. Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
