Project Description
Illinois State University is seeking statements of qualifications from engineering firms interested in providing design services for the installation of single mode fiber cabling to the inside plant of approximately 268 separate locations. Work will include connecting to an external network demarcation point and designing a path to the final equipment connection point. Solutions will vary from location to location. The estimated cost for this work is yet to be determined.

Effective January 1, communications regarding the specifications of this solicitation, which are not given in a public forum, are subject to public reporting. Questions regarding where the bid is to be mailed, the date and time of the opening or how to obtain a copy of the solicitation or any other procedural question will not need to be reported. The disclosure of the inquiries that need to be reported will include the name, address, phone number and email address of the person and firm making the inquiry. Any firm or individual who wishes to submit their qualifications in response to this solicitation must comply with the vendor requirements listed above, submit the State Board of Elections (SBE) certificate of business entity registration, complete and submit the Contractor Certifications and Conflicts Form (attached), complete and submit the Contractors Disclosures and Conflicts of Interest form (attached), and complete and submit the Vendor/Contractor Use of Subcontractors Form (attached). If you do not submit the SBE certificate (if required), the completed Contractor Certifications and Conflicts Form, the Contractors Disclosures and Conflicts of Interest form, and the Vendor/Contractor Use of Subcontractors form your qualifications may not be accepted by the University.
Your statement of qualifications must include the information requested above, the information included on the CDB 255 form, and any other information about your firm that you would like to include pertaining to this project. A copy of your CDB pre-qualification confirming your registration, the (1) Certificate of Business Entity Registration from the State Board of Elections, a copy of your (2) Contractor Certifications and Conflicts Form, (3) Contractors Disclosures and Conflicts of Interest form, (4) Vendor/Contractor Use of Subcontractors form, must be included in your response. Three copies of your statement of qualifications must be sent to Robyn Schuline, 1220 Purchasing Office, Uptown Crossing, 100 South Fell Avenue, Suite D, Normal IL 61761. Statements of Qualifications are due June 14, 2011, at 2:00 PM local time.

Only those firms which meet the following qualifications will be considered:
  • Submitted 3 copies of their qualifications.
  • Licensed to practice as an architect, landscape architect, or engineer in the State of Illinois.
  • Pre-qualified with the State of Illinois Capital Development Board.
  • Registered and authorized to do business in the State of Illinois.
Selection will be consistent with the Architectural, Engineering, and Land Surveying Qualifications Based Selection Act (30 ILCS 535/1).

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
