Illinois State University seeks a hosted web survey solution that can be utilized in research, the teaching of research design, and for general data gathering by the institution. The selected solution should be capable of integrating with existing email and web technologies and should provide both basic and advanced survey features including (but not limited to) branching logic, dependent fields, conditional workflows, time measured responses, etc. Additional preferred features of a web survey solution are full reporting options for general system status and dashboards for monitoring of individual surveys or groups of surveys. Lastly, a successful hosted web survey tool would adhere to the highest standards of data security and would be capable of protecting highly restricted data like that found in human subject research.

Submit 2 copies of the response as defined in the specifications. The University reserves the right to accept or reject any and all responses or parts thereof.
Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. There is no BEP goal for this procurement.
This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
