In an effort to simplify and eliminate the requirement that contractors prequalify repetitively during the calendar year, contractors who wish to prequalify for the period January 1, 2018 through December 31, 2018 can accomplish this by executing, in its entirety, the prequalification statement and all enclosures.

The Contract Certifications and Conflicts form, Disclosure and Conflicts of Interest form, Disclosure of Business with Iran form, Board of Elections Certification, Vendors Legal Authorization, references, and your company's substance abuse policy are also part of the prequalification process. This should eliminate the requirement of completing some of these forms with each project that is bid throughout the calendar year unless the form or the requirements are changed. The Disclosure and Conflicts of Interest form may be required to be completed on a project-per-project basis, but is still part of the prequalification process. SIUE does, however, reserve the right to request updated forms at any time and on a per project basis.

Prequalification with SIUE does not prequalify your company with CDB or any other Universities in the State of Illinois. CDB's prequalification statement will not be accepted on SIUE projects. In addition, SIUE reserves the right to prequalify for specific projects and not utilize the generic prequalification statement.

Companies will be notified if the document is approved or if additional information is required by SIUE. Companies will be notified only once requesting the additional information. As in the past, no bids will be accepted if a complete and approved SIUE prequalification statement is not on file with me during the current calendar year. E-mailing the completed prequalification is the preferred method of submission and the original document does not need to follow in the mail.

Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable.
