Amendment 1 to amend CDB pre-qualification documentation status. Vendors must be CDB pre-qualified or have application pending that will be complete and received by SIU School of Medicine on or before July 3, 2013 at 4:30P.M. Bid opening date remains firm of June 12, 2013 at 2:00P.M.
If your CDB Pre-qualification is pending, include a copy of the CDB pre-qualification application along with the Signed Amendment 1 Notice with your document submission for opening on June 12, 2013.

Project Description
The Board of Trustees of Southern Illinois University governing the Southern Illinois University School of Medicine hereby requests letters of interest and proposals, and statement of qualifications from architectural /engineering firms interested in providing professional consulting services for the programming, design and construction of the medical library renovations on the 4th floor of 801 N. Rutledge, Medical Instruction Facility. The selected firm would be used to design modifications to the existing medical library of some 23,000 square feet. The modifications include removing stack space and incorporating group study areas and technology appropriate for a state of the art library with new finishes and circulations that would be funded from university funds.

Qualifications and supporting materials will be accepted at the address below until 2:00 PM, Wednesday June 12, 2013:

Firms interested in providing such services should be:
  • A full-service architectural firm or team
  • Pre-qualified with the Capital Development Board (CDB) in all the architectural/engineering disciplines
The following related criteria will be among that used to evaluate the firms requesting consideration for selection:
  • Capital Development Board (CDB) prequalification status
  • Location of firm
  • Adequate staff size and disciplines
  • Prior performance of the firm and its consultants with completed projects at SIU School of Medicine and other State clients
  • Similar projects
  • Current workloads
Proposals should include a complete description of the history, size and capabilities of your firm and a list of projects which demonstrate your level of experience. For each project indicate a knowledgeable reference (name, address and telephone number; and a copy of your CDB Pre-Qualification Certificate. CDB 255 format is not required).
Firms should include:
  • A certificate of insurance conforming to the attached SIU School of Medicine insurance requirements
  • State Board of Elections Registration submission at time of submittal
  • Illinois Department of Human Rights (IDHR) number
Firms will be selected based on the criteria above and awarded to the most suitable firm.
The University reserves the right to accept or reject any and all responses or parts thereof. Illinois Department of Human Rights (IDHR) eligibility number is required on all responses. This procurement will be contingent on Board of Trustees approval.

Only those firms which meet the following qualifications will be considered:
  • Submitted 8 copies of their qualifications.
  • Licensed to practice as an architect, landscape architect, or engineer in the State of Illinois.
  • Pre-qualified with the State of Illinois Capital Development Board.
  • Registered and authorized to do business in the State of Illinois.
Selection will be consistent with the Architectural, Engineering, and Land Surveying Qualifications Based Selection Act (30 ILCS 535/1).

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
