The Board of Trustees of the University of Illinois on behalf of the University of Illinois at Chicago Department of Health Information Management is seeking Bids from qualified firms to store, retrieve, purge and destroy medical records from multiple locations for the period beginning on or about July 1, 2009 through June 30, 2012 with the option to renew for 2 additional twelve month periods at the same terms and conditions based on satisfactory performance, continuing need and availability of funds.
A pre-bid conference will be held at the University of Illinois Medical Center at Chicago, Health Information Management Department, 833 S. Wood on Friday, February 20, 2009 in Conference Room B-52 at 9:30 AM. Attendance at the pre-bid conference is highly recommended as representatives of the University will be present to answer any questions regarding the services requested or bid procedures. Prospective Bidders should respond via e-mail at shorter@uillinois.edu or 312-996-2850 with firm's name and number of attendees. A maximum of two representatives from each firm may attend.

The University reserves the right to accept or reject any and all responses or parts thereof.
Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents.
Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.
This procurement will be contingent on Board of Trustees approval.
This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
