The University of Illinois at Chicago on behalf of the Department of Medicine invites sealed bid proposals for the provision of sleep lab supplies for the period beginning March 15, 2010 and ending March 14, 2011, with the option to renew for four (4) additional one (1) year periods as described herein.

The University reserves the right to accept or reject any and all responses or parts thereof.
Vendor may expect payment thirty to sixty days after receipt of invoice, unless otherwise stated in the bid documents.
Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals.
This procurement will be contingent on Board of Trustees approval.

This transaction is subject to the State Board of Elections registration and certification requirements of Public Act 095-0971.
