This amendment authorizes additional design services for Floors 2,4, and 5 through 50% Schematic Design.

Project Description
The University of Illinois at Chicago is seeking a Professional Services Consultant (PSC) for the design and construction of a new Ambulatory Surgery Center (five story plus MEP penthouse) to be located at the SE corner of Taylor & Wood Streets, Chicago, IL., with a two-story bridge to the University Hospital on the north side of Taylor Street. Interior improvements include a 22,500 GSF surgery center with operating rooms, staff and administrative areas, sterile processing, and pre- and post-care facilities, on the 3rd floor. An imaging suite, lobby, reception area, and receiving facilities on the 1st floor. The remaining spaces will be shelled space for future expansion. The initial phase of the project will include conceptualization and 50% schematic design; including an analysis and feasibility of sustainable systems that may assist UIC with its climate commitments. The conceptualization phase will establish the scope of the work. After a successful conceptualization/50% schematic design phase, the PSC may be authorized to proceed with the ensuing phase(s) including, but not limited to, remaining schematic design, design development, construction documents, bidding/award, construction administration & post construction, following The University of Illinois Board of Trustees' approval. The University will hire a Construction Manager to provide pre-construction & construction services. The project will employ a fast track construction schedule. The selected PSC will be required to prepare multiple bid packages for bidding, with the first bid package released no later than Aug 3, 2018 and the last bid package released no later than Nov 9, 2018. Project completion by Dec 31, 2019. The project will be designed to be certified LEED Gold.
