Change Order Amount: $6,378.75
This change is to remove 550 sq. ft. of additional concrete sidewalk. Install 550 sq. ft. of 5” concrete sidewalk with tooled control joints and fiber-mesh reinforcing. Increase the thickness of the proposed sidewalk on the west side of the library from 4” to 5” and add fiber-mesh reinforcing. WIU would like to proceed as quickly as possible prior to the students returning to use the library entrance.

Original construction of the library walls was very poor, and the walls have deteriorated to a point of instability. These are not adequate to affix the new stone to as was the project plan, and will need to be replaced. This will replace all three walls with concrete.

The scope of this project involves removing the existing brick at the entrance walls on the east and west side of Malpass Library on the campus of Western Illinois University, Macomb, IL, and replacing it with cast stone.
A pre-bid meeting will be held on Thursday, April 5, 2012 at 10:30 am in WIU's Grigsby Physical Plant small training room, Macomb, IL. A site visit will be held immediately following the pre-bid meeting.
Any award will be subject to the Responsible Bidder provisions of the Illinois Procurement Code, including, but not limited to payment of prevailing wage, utilizing equal employment opportunity hiring practices in connection with the project, and participating in applicable apprenticeship programs. Bids that do not include a current Board of Elections registration form will be rejected.
