CSU is seeking offers from qualified offerors to provide athlete accident insurance coverage, plan administration, and claim processing services according to the requirements of HIPAA and FERPA. (See Attachment A). The University is a member of the National Collegiate Athletic Association (NCAA), with approximately 175 participating student athletes. See Attachment AA for NCAA requirements.

The University reserves the right to accept or reject any and all responses or parts thereof. Illinois Department of Human Rights (IDHR) eligibility number is required on all proposals, if applicable. This procurement may be contingent on Board of Trustees approval.
